Athletics Resources and Forms
(Required) Athletics Registration & Payment Form
EACH participating athlete (excludes Enrichment programs and Clubs) must register for EACH Compass. A Registration & Payment form must be submitted for each child and each sport in which they plan to participate.

(Required) Athletics Release Form
Every participating athlete (excludes Enrichment programs and Clubs) must fill out the ATHLETICS RELEASE FORM. This form is required for Compass but also includes the behavior expectations for students, coaches, and parents for any Jeffco League sport. This is not your registration form.
(Required) Physical Form
This Physical Form can be filled out by your doctor to show proof of physical and submitted to [email protected]. You can also submit the physical paperwork electronically. Physicals must be within one year of the start the season in which your child is planning to participate.
