***REGISTRATION NOTES***

FORMS: You are required to submit an up to date copy of your child's physical (must be current within one year of the start of the camp). You can use our form or submit your physical that states the child is cleared to play sports. As with any Compass sport, you need to submit the Athletics Release form once for the entire year.

LIABILITY: By submitting this camp registration form, you also acknowledge that participation in this camp requires stunts and tumbling activities, which could cause injury. You agree to not hold Compass Montessori or it's staff/coaches liable for any injuries, should they occur. You accept that the sport is risky an

FEES: The fee includes your uniform, which must be cleaned and returned at the end of the season. If you did not attend the camp, then you must purchase pom poms for an additional $15. If you already have pom poms from camp, then do not purchase them here. If you would like to buy the uniform at the end of the school year, we can bill you for it. 

REMINDER: YOU MUST HAVE A VALID PHYSICAL TO PARTICIPATE

COMPASS PROOF OF PHYSICAL FORM: You must use the Compass form for proof of physical or one from your doctor. You can print THIS COMPASS FORM and have your child's doctor sign and return. You can then upload the signed copy to this registration form in the section below. The ONLY accepted forms are the Compass form or the Doctor's form, which clearly states that your child is healthy enough to participate in sports/activities. Your child will not be able to participate without a proof of physical.
